We are in the process of planning our next annual survey and would be interested to know if you have any suggestions about what we should be consulting our patients about.
The following list gives some suggestions of the areas that you may like to see included in the survey. We would like your opinion on whether you feel they should be included and for you to give any additional comments/suggestions or specific questions you would like asked.
We would appreciate your reponses by Friday 23 December if possible.
